Not Registered with a Doctor?

If Client has a permanent address they can register with a G. P. practice of there choice:
a)         Go to practice and register in person
b)         Fill in G. P. Assignment Request Form and fax to Co. Durham Health Authority.
           Fax : 0191 3333959
If Client has a temporary address i.e. Bed & Breakfast they can register with a G. P. practice of their choice for up to 3 months. Process is the same as above.
If client is street homeless or ‘couch hopping’ they cannot register with a G. P. but they can go to any doctor and ask for urgent medical attention.

In practice the worker should:-

Phone surgery up to request an appointment and with the client’s permission give details of what the health problem is.
Wherever possible accompany the client to the surgery.
